Individual Support Services assist with acquiring, retaining, improving and generalizing the behavioral, self-help, socialization and adaptive skills necessary to function successfully in the home and community. Individual support services are behavioral, self-care and habilitative related tasks performed and/or supervised by service provider staff in a family home, the home of a relative or in other community-based settings, in accordance with approved treatment plans.
Activities of Daily Living (ADLs) are defined as needed skills related to daily self-care activities within an individual’s place of residence, in outdoor environments, or both.
Basic ADLs (BADLs) skill building (ages 21+
): BADLs consist of self-care tasks, including but not limited to:
Bathing and showering
Dressing
Eating
Personal hygiene and grooming (including washing hair and brushing teeth)
Toilet hygiene
Instrumental ADLs skill building (age 16 and over): Instrumental activities of daily living (IADLs) are not necessary for fundamental functioning, but they enable an individual to live independently in a community and include but are not limited to:
Housework
Taking medications as prescribed
Managing money
Shopping for groceries or clothing
Use of telephone or other form of communication or Using technology (as applicable)
Transportation within the community
Services that provide direct support and assistance for participants, with or without the caregiver present, in or out of the participant’s residence, to achieve and/or maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community, as outlined in his/her Service Plan. Community-Based Supports are delivered one-on-one with a participant and may include but are not limited to: assistance with community-based activities and assistance to, as well as training and supervision of, individuals as they learn and perform the various tasks that are included in basic self-care, social skills, and activities of daily living.
Services provided outside of participant’s home that support and assist participants in educational, enrichment or recreational activities as outlined in his/her Service Plan that are intended to enhance inclusion in the community. Community Inclusion Services are delivered in a group setting not to exceed six (6) individuals. Examples: Going to the movies, concert, go votes, go watch games etc.
